The published schedule amount is the reference for rental months 1–3. For E0260, months 4–13 use 75% of that amount. Select a month to see that separate calculation.

Sources, dates and geography limits
CMS DME26-C is the July 2026 snapshot. This preview accepts service dates from July 1 through September 30 only; it does not project fees into later quarters.
ZIP matching uses the CMS Former CBA and rural ZIP lists. These are not a complete directory of all US ZIP codes. If neither list matches, the area remains unresolved. Manual state selection is a separate scenario, not a verified ZIP result.
The rental calculation follows DMEBACK 2026.pdf, section I, CAPPED RENTAL ITEMS, printed page 3. Preview rounding uses decimal half-up to cents and has not been checked against claim adjudication.
