E0260 Medicare fee schedule: dates, ZIP areas and rental months

Understand the reviewed Q3/Q4 2026 reference before using the lookup.

What an E0260 fee reference tells you

E0260 describes a semi-electric hospital bed with mattress and side rails. This guide explains the E0260 rental schedule reference used by DMEAtlas for July–December 2026.

A schedule amount is not a coverage decision, a claim payment or your final bill. Check the supplied equipment, applicable policy and supplier terms separately.

1. Select the snapshot by date of service

Use the service date, not the date you open this page. DMEAtlas currently supports these two fixed snapshots:

The service-date window, source observation date and human review date have different meanings. A review date does not make a policy effective on that date. Dates outside these windows are not supported by this collection.

2. Understand the CMS fee area

The lookup uses the CMS rural and Former CBA ZIP lists bundled with the selected quarter. These lists are not a complete postal ZIP directory. A ZIP without an unambiguous match does not receive an inferred fee.

CMS fee-area names are not postal city or county names. For example, ZIP 10001 maps to the source short name “Bronx, NY”; the complete CMS area name in both files is “Bronx-Manhattan, NY”. This is a fee-area label, not a statement that the ZIP is in the Bronx.

If you choose a state and rural/nonrural area manually, the result is an unverified scenario. It does not validate the ZIP or determine which area applies to a claim.

3. Keep the rental-month calculation separate

E0260 is imported as rental modifier RR and payment category CR. For this capped-rental reference, the published amount is used for months 1–3; months 4–13 use 75% of that amount. CMS describes this relationship in its DMEPOS payment policies, Capped Rental Items section.

Enter a known month of continuous rental. DMEAtlas does not determine rental continuity, interruptions, ownership transfer or claim eligibility. Its calculated examples round decimal half-up to cents; this is a display convention, not claim adjudication.

Each row uses the selected quarter's ZIP mapping and fee record. The fourth-month example is $128.14 × 0.75 = $96.105, displayed as $96.11. Do not assume that all ZIP codes in the same state have the same amount.

4. Ask for your own cost explanation

Medicare.gov explains that Original Medicare Part B cost sharing for covered hospital beds generally includes the deductible and 20% of the approved amount when the supplier accepts assignment. Other insurance and supplier terms can affect what you owe. A displayed schedule reference does not calculate your personal balance.

Before delivery, ask the supplier to explain the supplied configuration, rental month, assignment, expected cost sharing and any noncovered upgrades. For Medicare Advantage, ask your plan about its own arrangements.
